[Dev] Chandler Code Review Guidelines

Hi,

I created and posted a first shot for Code Review Guidelines on the Wiki:

My first intent was to cover things we (App team) need to do for 0.6 (as 
far as code review is concerned) but the doc is a little more general 
than that and also covers critical patches and external submissions. Let 
me know what you think.
