[Design] Mark-up bar button visual treatments

Here are 4 different treatments for visual feedback on mark-up bar  
buttons. These are rough mock-ups. Sizing and exact coloring still  
need to be tweaked. I wanted to throw some stuff out and narrow the  
field of options a bit.

 From left to right, the states are: Active, Rollover, Mousedown,  
Selected

In all 4 proposals the Rollover and Mousedown states would look the  
same for both Active and Selected states.

Why there are no "color" options right now.
+ Interaction states need to be readable on their own, without color  
for accessibility reasons.
+ Want to nail down a general approach before applying color.  
(Comparing color treatments and monochrome treatments would be like  
comparing apples and oranges.)

+ In the long run, it may be too cluttered to have color in the mark- 
up bar.
++ Imagine the main toolbar filled with big colorful icons
++ The calendar filled with big colorful event lozenges
++ A table dominated by big colorful triage status column
++ A big colorful triage status button in the mark-up bar itself
