As a naive user, I think it means what it says it does, i.e., #1

If you want to do #2, you shouldn't say "iCal" under the button but 
something to the effect of "other application" and get a UI to display a 
list of supported CalDAV enabled apps.

#3 is a different and interesting feature that might be worth another 
different widget/affordance.
