Sounds great.

Will each version contain only the latest notes, or a running  
accumulation of previous releases as well?

One alternative is a simple README which has an essential summary of  
the package, purpose, authors, pointers to resources, etc.  That is, a  
README which doesn't change much.  And a separate file (Changelog,  
CHANGES, RELEASE-NOTES, etc) with specifics for that release.

What you committed is great and a clear improvement.
