i've updated the page to list the various urls that will be exposed in
the web ui page's document head and in the APP service document
(previously called introspection document).

i also introduced the term "webcal" to describe the monolithic
icalendar view of a calendar collection that we use for ical and
google interoperability and provided a separate url for webcal access.
GET on the dav url will return the bare bones html "collection index"
that cosmo serves today for non-calendar collections. separating the
webcal and dav urls reduces the amount of special case code in the dav
protocol handler.
