Esther's problem was there was an old item whose icalUID matched another 
old item.  I'm guessing that item's been in a conflicted state for 
months and only became a problem when I suggested removing local sharing 
state, which removes conflicts.

It looks to me like the problem was caused by Bug 10881, the event in 
question precedes the fix for that bug so I'm guessing (hoping) this is 
a one-off problem, we're not likely to see it again.
