This should all be straightforward to implement. There should probably  
be keyboard equivalents (and/or mnemonics) for the new menu items in  
#3. Personally, I like the straightforwardness of Cmd/Ctrl-R for  
reply, Cmd/Ctrl-Shift-R for reply all, and Cmd/Ctrl-F for forward.
